2. Create Username (first time):
https://medicalmarijuana.colorado.gov/state-co-entellitrak/page.request.do?page=page.public.userRegistration
3. Complete and Submit Application Online. Your Doctor will submit the Physician Certification.
Or if you do not/can not apply online:
Apply by mail
It will take approximately 6-8 weeks to get your card if you apply by mail.
Complete the following documentation and submit to the registry through certified mail.

- Cancer
- Glaucoma
- HIV or AIDS
- Cachexia
- Persistent muscle spasms
- Seizures
- Severe nausea
- Severe pain
- Post Traumatic Stress Disorder (PTSD)
- An Autism Spectrum Disorder
- Any condition for which a physician could prescribe an opioid.
- Physician must be in good standing and licensed to practice in Colorado. Contact Colorado Department of Regulatory Agencies for information about physician licenses.
- Physician must be an MD or a DO.
- Physician is registered to submit certifications online.
- If you or your physician is not online, they must provide you with a paper copy of your medical marijuana certification.
